Application of Ultrasonography. A healthy adult standardized patient lies on a stretcher. An examiner sits at the machine and changes settings only on your request; linear, curvilinear and phased-array probes are available. You hold the probe and may reposition the patient. Three tasks follow in order, a vascular access image, a nerve block image and a POCUS view, each limited to 2 minutes 30 seconds.2

Treat the model as Mrs. Brennan, a 79-year-old woman, 60 kg, on dialysis, who needs open fixation of a right bimalleolar ankle fracture. She was hypotensive to 84/46 after her last dialysis session this morning. The team wants a right internal jugular line, a popliteal sciatic block for the ankle, and a quick look at her volume status.

Your task: for each request, choose the probe, position the patient, ask for the settings you need, and narrate the image and your endpoint.
