You are an attending anesthesiologist. Over the past three weeks you have noticed several things about a colleague, Dr. Evan Hale. His anesthesia records twice showed fentanyl documented that did not match what he dispensed. His patients arrive in the PACU with high pain scores despite large documented opioid doses. He has repeatedly volunteered to give breaks so he can relieve colleagues. This morning you relieved him for lunch and found him drowsy at the workstation with pinpoint pupils. He is now anesthetizing a patient in OR 6.

You have asked to speak with the department chair, Dr. Morgan, who is in her office.

Your task: present your concerns to the chair and agree on next steps.
